DescriptionPosition Title: Nurse Practitioner/Physician Assistant

Primary Location: Harwich Port

Employment Status: Exempt

Reports To: Medical Director
Essential Functions

Assess patient wellness/illness by obtaining complete medical history ensuring that findings are properly recorded in patient's medical record.
Perform physical examinations, orders laboratory tests and analyzes results to evaluate acute/chronic complaints.
Diagnose illness or health needs based on analysis and evaluation of clinical data collected; plan and provide treatment according to protocols and/or consultation with supervising physician and/or Medical Director.
May concentrate in such medical specialties as pediatrics, gynecology, internal medicine, family practice, and family planning, etc.
Provide consultation to nurses who triage patient telephone calls and walk-ins; refer patients for specialty services after consultation with physician.
Collaborate with others to plan, develop and evaluate new patient care and community health programs and policies; participate in OCHS quality assurance activities.
Maintain and enhance professional knowledge of theory and practice by attending CME conferences, educational programs and seminars as per licensure requirements.
Perform minor surgical procedures under the supervision of a physician.
Track patients through follow-up visits and continuing contact.
May serve in the on-call schedule rotation, requiring night and weekend hours.
Adhere to OCHS policies and procedures.
Other related duties and projects as required to meet expectations of position.

QualificationsRequired Qualifications, Education, and Experience: Nurse Practitioner

Bachelor of Science in Nursing required from an accredited school of nursing
Position requires a current license from the Massachusetts State Board of Nursing Registration
Nurse practitioner certification
Certification in cardiopulmonary resuscitation (CPR)
BLS certification required
2 years' experience as a nurse practitioner in a community health/clinic environment
4 years' of nursing supervisory experience.

Required Qualifications, Education and Experience: Physician Assistant

Bachelor's degree or equivalent in the biological or physical sciences
Successful completion of a two-year accredited Physician's Assistant Program
Current certification to practice as a Physician's Assistant by the Massachusetts Board of Medicine
Certification in cardio-pulmonary resuscitation (CPR)
Current BLS and ACLS required
2 years' experience in a clinic setting as a Physician Assistant
