About this position as a Pedi Pal Registered Nurse (RN)

The Pediatric Palliative Care Network (PPCN) is designed to meet the needs of children with life limiting illnesses and their families in the Plymouth, Norfolk and Bristol county service areas. The Pediatric Palliative Care nurse will be knowledgeable about the PPCN program and its operation. The nurse performs skilled nursing visits for PPCN regardless of reimbursement resource. The nurse will be a resource to staff and other referral sources. They will adhere to system and department compliance policies, and any and all applicable laws and regulations. They will performs other duties as assigned.

This position is a part-time 20 hours per week with the hours of 8:00am-4:30pm

Provide comprehensive medical evaluation of patients and families in their homes
Pain and symptom management
Collaborative goal setting and coordination of care among the other members of the child/family care team
Attend weekly Interdisciplinary Team meetings
Create and update care plans for each patient
Provide education to the families about medications, treatments, procedures, nutrition, safety, and disease process
Provide family with community resources
Participates in special events for families
Understand the dying process and manage the emotional impact of working with the dying and bereaved
Communicate regularly with the Pedi Pal Program Manager to feedback progress or concerns regarding any aspect of clinical work and to structure workload appropriately
Clinical expertise and competency in the practice of standard precautions and agency infection control practices
Help bring quality of life to our Pedi-Pal Patients.
Have a strong understanding of conditions, causes and treatments of a variety of disorders.
Verbal and written communication skills needed to communicate clearly, effectively, and patiently on the phone and in email/text messages
Proficiency with computers, Zoom and healthcare databases.
What are the qualifications for this job as a Pedi Pal RN?

Pediatric Nursing Experience required.
Hospice or Palliative Care Experience preferred.
Nursing Degree, Associate's, BSN or MSN
Bereavement and/or end of life care preferred but not required
Home care preferred but not required
Knowledge of special needs and behaviors of infants, children, and adolescents
Current MA state RN license
Valid driver's license with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.
Certified Hospice and Palliative Nurse (CHPN)-preferred but not required
Must be able to function independently in the home care setting while maintaining team collaboration.
Must be able to provide assessment and interventions to children from a diverse background of ages, needs and abilities
Must be knowledgeable about a wide variety of health care experiences that pediatric palliative care patients may encounter.
Ability to read and comprehend instructions and correspondence in English.
Ability to clearly present information effectively to varying levels of staff and management physicians, patients, administrative assistants, operations managers and supervisors and other support and professional staff.
Ability to apply mathematical concepts such as fractions, percentages, ratios, and proportions to practical situations.
