JOB SUMMARY : The Licensed Practical Nurse (LPN) collects patient/family information that assists the Registered Nurse in assessment, planning, implementation, and evaluation. The LPN provides safe, effective care for assigned patients with the assistance and under the supervision of a Registered Nurse.

This position is a

Safety Sensitive Position . The essential functions, physical demands, and mental competencies of this job require the employee maintain the ability to work in a constant state of alertness in a safe manner.

JOB SPECIFIC DUTIES AND RESPONSIBILITIES:
Obtains, verifies and updates patient information and provides support services to patients and medical staff.
Requests, locates, sends and receives patient medical records.
Loads labs, medications and other patient information into the electronic medical records (EMR).
Assists in exam rooms.
Assists provider(s) with examination, diagnostic procedures and treatments.
Provides direct patient care as ordered.
Answers and triages phone calls.
Completes insurance and disability paperwork.
Documents all interventions, patient/family responses, medication dispensed or prescribed and hospital data in the medical record.
Promotes wellness by providing patient education materials, communicating provider's advice and instruction.
Coordinates patient care by triaging patient depending on urgency of patient's needs.
Administers injections / medications and documents all necessary information ordered by the provider.
Adheres to safety guidelines by maintaining equipment and instruments.
Stocks supplies.
Prepares Patient Occurrence Report/Variance Reports as needed.
OTHER JOB REQUIREMENTS : May be required to travel to satellite clinics.

EDUCATION, CERTIFICATION, LICENSURE and REGISTRATION:

Graduate of an accredited school of practical nursing.
Current LPN License issued by the Arkansas Board of Nursing.
BLS Certification.
EXPERIENCE, KNOWLEDGE, SKILLS and ABILITIES:

Minimum one year LPN experience.
Excellent computer and other basic office equipment skills.
Time management and organizational skills.
Attention to detail skills.
REASONING ABILITY:

Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists.
Ability to interpret a variety of instructions furnished in written, oral, or schedule form.
INTERPERSONAL SKILLS:

Must interact and communicate both verbally and in written form.
Must interact and exchange information regarding patients with providers, other departmental personnel, and outside agencies on a frequent basis while respecting the confidentiality of patient information.
PHYSICAL DEMANDS:

The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.

While performing the duties of this job, the employee is regularly required to talk or hear.
The employee regularly is required to stand, walk, sit, use hand to finger motion, handle or feel objects, tools, or controls; and reach with hands and arms.
Specific vision abilities required by this job include close vision and the ability to adjust focus.
