Must currently hold an active unencumbered license within the State of practice; or have a temporary permit allowing you to practice in the State; or have applied for State LPN licensure by endorsement; or be a new graduate in the process of taking your LPN exam.

- Provide general nursing care to residents in coordination with a multidisciplinary care team.- Contribute to overall resident care guided by care plans, facility policies, and professional standards of practice.- Supervise Nursing Assistants directly and coordinate the care of assigned residents, consulting with the Director of Nursing/Charge Nurse as necessary.- Assist in interpreting and implementing facility policies for staff.- Ensure overall resident care activities align with department policies, procedures, and standards of the profession.- Provide care within the boundaries of physician orders, department procedures and policies, and LPN scope of practice.- Contribute to nursing assessment, participate in the development of nursing diagnosis, care planning, and implementation of nursing interventions.- Evaluate responses to nursing interventions.- Facilitate outside physician visits to ensure residents' medical needs are met.- Demonstrate effective communication with physicians and family members to ensure residents' needs are met.- Conduct teaching sessions for residents, families, and staff to ensure understanding of residents' care plans.- Promote and implement the choices philosophy and resident rights.- Prepare, administer, and chart routine and emergency medication and treatment as ordered, demonstrating knowledge of medications and their side effects.- Ensure clinical records reflect accurate and timely documentation to ensure continuity and quality of resident care.- Perform safety and infection control procedures to ensure a safe and clean environment for residents and staff.- Participate as a member of the interdisciplinary team in the Quality Management program to ensure appropriate standards of care are met within the facility.- Function as the primary contact person on assigned shifts.- Effectively supervise resident assistants to ensure compliance with the plan of care.- Contribute to the physical, emotional, and spiritual needs of individuals, families, and significant others within the care environment.
