Provide medically necessary support and education with the caregiver(s) of the consumer. The intent of the service is to maintain the consumer in their home and community. The service is delivered according to the consumer's HCBS SED Waiver plan of care. It is time limited and goal directed

Actively participates in outstanding customer service and accepts responsibility in maintaining relationships that are equally respectful to all.

ESSENTIAL FUNCTIONS:

* Build a positive, non-judgmental and culturally competent relationship with the consumer's caregiver(s)

* Knowledge of the Horizons Mental Health Center Home and Community Based SED waiver policies and processes

* Delivery of the Parent Support and Training service either individually or in a group setting which is determined in the consumer's plan of care

* Knowledge of the caregivers' concerns and needs for information regarding the nine domains addressed by the strengths and needs assessment of the consumer's plan of care

* Document the content of each service provided according to the established policy of the agency and with the use of the Electronic Medical Record.

* Assist the caregiver(s) in collaborating with service providers

* Serves the primary caregiver(s) of the identified consumer

* Maintain boundaries in all interactions with caregiver(s) and staff

* May perform other duties as assigned or requested

MINIMUM KNOWLEDGE AND SKILLS REQUIRED:

* Ability to maintain timeliness for appointments

* Prioritization and scheduling of appointments for individual's on assigned caseload

* Function as a member of the consumer's team

* Demonstrated ability to communicate effectively

PREFERRED KNOWLEDGE AND SKILLS:

* Previous experience of working with caregivers of a child / youth identified as having a serious emotion disturbance.

* Knowledge and experience with the various systems involved in a child's / youth's life.

* Knowledge and experience of working in a culturally sensitive, non-judgmental and strengths based family centered system of care.

MINIMUM EDUCATION AND EXPERIENCE REQUIRED:

* Have at least a BA/BS degree or be equivalently qualified by work experience or a combination of work experience in the human services field and eduction with one year of experience substituting for one year of education.

* Must be at least 21 years of age, with a minimum of two years' experience working with children with serious emotion disturbance.

PREFERRED EDUCATION AND EXPERIENCE:

* Experience working with adults.

* Preference is given to parents

* Experience or education on providing strengths based care.

LICENSURE OR REGISTRATION REQUIRED:

* Valid driver's license, insurance and a dependable vehicle
