Internal Medicine Nurse Practitioner

experienced APP (APRN or PA)

to oversee the

West Volusia County Transitional Care Clinics.
We are seeking an APP, with a

team player mentality , and a

dedication to continuing our tradition of excellence

in both clinical practice and professionalism, while maintaining AdventHealth s mission, vision, and values.
The position will require the ability to manage complex, high risk, high frequency patients with

the goal of reducing inpatient readmissions . The APP will collaborate with an on-site pharmacist, respiratory therapist, social worker (community care), and diabetic educator, as needed.
The opportunity has a

4- (10) Hour weekday schedule , with clinic locations in close proximity to our

AdventHealth Deland

and

AdventHealth Fish Memorial

campuses.
Requirements of the position:
3+ Years experience
Board Certification: Board Certification: Physician Assistant or Nurse Practitioner
Active FL medical license (or ability to secure active, unrestricted licensure)
DEA License required (or ability to obtain)
Strong communication skills
The practice is a part of AdventHealth Medical Group.
