Position Overview:
The MDS Coordinator is responsible for overseeing the Minimum Data Set (MDS) process in the nursing facility, ensuring compliance with federal and state regulations. This role involves assessing, documenting, and coordinating care for residents, as well as collaborating with multidisciplinary teams to ensure high-quality care delivery. The MDS Coordinator will also manage data and assist with care planning for residents to support the facility's operational objectives.

This position provides care to assigned residents in a caring, safe, and efficient manner and is responsible for performing the following according to location standards, procedures, and individualized resident care plans. This role is crucial in maintaining the integrity of resident assessments and ensuring they receive appropriate care.

Key Responsibilities:

Complete assigned portions of the MDS for nursing, sign forms to signify the completion of the process, and maintain residents' self-esteem, privacy, and confidentiality.
Assist in maintaining safe, pleasant, and orderly resident living areas.
Complete nursing portions of MDS and review for completion and validity of the entire process.
Query other team members for accuracy of MDS and perform final quality checks prior to MDS transmission.
Observe and report residents' conditions, and communicate with physicians using center/campus systems.
Direct and assist with co-worker, resident, and family/significant other education.
Provide support and recognition to co-workers related to completion of the RAI/MDS process.
Use the Electronic Medical Record (EMR) system accurately and consistently.

Qualifications:

Graduate of an accredited school of nursing with an active RN nursing license for the state of Iowa.
Previous experience in an MDS coordinator role or similar clinical setting is preferred.
Strong knowledge of the RAI/MDS process, basic nursing skills, and regulatory compliance.
Ability to communicate and comprehend effectively, both orally and in writing.

Work Environment:

The noise level in the work environment is usually moderate.
Weight Lifting Requirement: Up to 25 lbs.
