Responsibilities
Provides consultative services and follow up visits for hospice and palliative care programs.
Determines patient's initial and continuing eligibility for hospice care and writes certificates of terminal illness for admitted patients.
Provides expertise regarding pain and symptom management to all referred patients.
Acts as a resource to community physicians and clinicians regarding symptom management facilitating their understanding of hospice and palliative care.
Consults with appointed attending physician and coordinates medical care provided by the Hospice IDT team for their patients, as director of that team (home, nursing home, or Community Hospice House).
Shares in on call duties; one weekday a week from 5pm to 8am and one weekend approx. every 4th/5th from 8am Saturday to 8am Mondays.
Preferred Qualifications

Board certified in hospice and palliative care.
Required

MD/DO degree from an accredited institution.
Physician licensed to practice medicine in New Hampshire with minimum courtesy privileges at hospitals.
Unrestricted DEA provider registration and Controlled Substance License.
Comprehensive knowledge of palliative care with special expertise in pain and symptom management, related pharmacology and in the psycho-social aspects of the care of the terminally ill patients and their families.
Comprehensive knowledge of primary care medicine with ability to develop collaborative relationships with physicians at area hospitals.
CPR Certified.
Satisfactory criminal record.
