Hospice Liaison RN, Weekend Lead
Full Time, Tuesday through Saturday
On site in Farmingdale, NY
The
Hospice Liaison, Weekend Lead
is responsible for assessing, planning and developing referrals, establishing and maintaining professional relationships with key physician groups and referral sources. This position is also responsible for coordinating patient transfers to our two Hospice Inpatient Centers. This position will work closely with our Business Development Manager and assume leadership responsibilities in their absence.
The ideal candidate will possess excellent interpersonal, marketing and customer service skills, and act in a professional and supportive manner at all times.
Minimum Requirements:
Current and valid NY State license to practice as a Registered Nurse
Bachelor's degree in Nursing
Minimum of three years' acute care nursing experience, preferably in Hospice, Palliative Care and/or Oncology
Strong computer skills, including Electronic Medical Records. EPIC a plus
Posted Salary Range
USD $100,000.00 - USD $120,000.00 /Yr.
This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate's qualifications, skills, competencies and experience. The salary range or rate listed does not include any bonuses/incentive, differential pay or other forms of compensation that may be applicable to this job, and it does not include the value of benefits.
