Coverage Area:

Northeastern Lake County Illinois
Great pay at $75/hr ($3,000/wk) with weekly direct deposit!
HH Case Manager RN Contract
13 weeks with extension options

HH Case Manager RN Shift

Day shifts
8 hour shifts (8:00 am-4:30pm)
some weekend rotation possible
No on-call
Required Certifications

: BLS, IL RN license
Experience:

1 year Home Health RN experience
Charting System

: EPIC (experience required)

Patients requiring Home Health, Infusion and Pediatric nursing, and/or Palliative and Hospice services.
Conducts initial assessment
Establishes the plan of care, Provides direct patient care, Provides on-going assessment of patient response to treatments and teaching and applies interventions as appropriate. Updates and revises plan of care as indicated.
Completes documentation including visit assessment, OASIS, 485 and verbal, Communicates with physicians to confirm and update patients' plan of care, Facilitates the discharge planning process according to standards and utilizes resources to assure continuity of care after discharge.
