Overview
PRN NP supporting annual wellness visits for Medicare/Medicaid patients in their homes. We seek an NP PRN status to work 8-12 hours per week or 32 hours per month. Role comes with A+ rated mal-practice, 401K with match, and salary of $145.00 per in-home visit, $85.00 for telehealth, and $20.00 for no-call no-show.

ResponsibilitiesThe Nurse Practitioner In-Home Assessment are responsible for performing Health Risk Assessments and occasional transitional care for patients discharged from hospitals to prevent re-admissions. As a Nurse Practitioner In-Home Assessment you will:
