Title: Licensed Clinical Social Worker (LCSW)

POSITION SUMMARY:

This is highly responsible and independent position. The Licensed Clinical Social Worker (LCSW) works as part of a multidisciplinary team to case manage a panel of patients who require mental health and substance abuse services. Services include assessment, diagnosis and treatment planning of behavioral health for children, adolescents, adults, and families in order to improve psychosocial functioning. Works closely with the Psychiatric Prescriber, Psychologist and Registered Nurse on behavioral health issues.

MINIMAL QUALIFICATIONS:

Master's degree in Social Work and Florida license in Clinical Social Work
Minimum of two years' work experience as a LCSW
Must have the ability to work effectively independently and in a team environment
Excellent interpersonal skills
Excellent verbal and written communication skills
Requires professionalism and ability to deal effectively with patients, families, caregivers and a variety of medical professionals
Bi-lingual English/Spanish, preferred

RESPONSIBILTIES AND PERFORMANCE EXPECTATIONS include, but are not limited to, the following:

Addresses referrals from providers.
Provides therapeutic services to patients and their families. Observes client behavior, assesses needs and develops treatment plans and strategies.
Diagnoses psychological, behavioral and emotional disorders.
Completes required documentation and paperwork in a timely manner. Maintains accurate and timely records, including clinical progress notes, according to organizational policy during the course of patient's treatment.
Consults with doctors, psychologist and medical professionals when appropriate.
Develops and maintains thorough knowledge of referral resources throughout the service area and acts as advocate to help patient gain access to such resources.
Maintains up to date knowledge of Medicare, Medicaid and other third-party reimbursement sources. Administers social service programs.
Participates in community service and health promotion programs. Participates in quality management activities as necessary. Responsible for ongoing staff and provider education and updates to committees on Behavioral Health Integration Project.
Provides positive customer relations in dealing with patients, families, physicians, member of the healthcare team, referral sources, services agencies and others.
Complies with Risk Management, Safety and Compliance programs. Maintains confidentiality of patient and patient records as required under HIPAA.
Rotate to other CFHC centers as needed to provide coverage if requested.
Report as needed to DOBH any pertinent information or situations that may impact on patient care or CFHC liability.
Attend and participate in mandatory CFHC meetings (as designated by the DOBH).
Maintain neat and professional appearance at all times.
Participate in staff outreach and off-site health care programs as requested by the DOBH, CMO, or CEO. Represent CFHC in the community.
Report as needed to DOBH, CMO and CQO any pertinent information or situations that impact on patient care or CFHC liability.
Carry out any other activities deemed appropriate by the DOBH, CMO, or CEO.

PHYSICAL REQUIREMENTS:

3. Ability to lift weight equivalents to what would be required when occasionally assisting and positioning patients, re-positioning equipment and lifting supplies

4. Ability to adapt and function in varying environments of workload, patient acuity, worksites, and work shifts.
