Documentation meets criteria for core measure, charges, PI, and all quality/regulatory

standards as measured by chart audits.

Assists in completing, monitoring, and evaluation of unit-based Performance Improvement

activities.

Keeps abreast of pertinent changes related to clinical information from CMS and other

regulatory agencies.

Initiates orders with physician countersign.

Collection of comprehensive data pertinent to the patients health/situation and unit specific

data for process improvement initiatives including initial patient assessment within expected

timeframe, initiates consults as indicated from database collection, identifies patient/family

teaching and discharge planning needs.

Analyzes the assessment data to determine expected outcomes and plan of care

The patients plan of care is developed, individualized, and prioritized daily in collaboration

with the patient, family and health team members that is sensitive to the patients cultural and

spiritual needs and to attain expected outcomes.

Evaluates progress toward attainment of expected patient outcomes and unit specific process

improvement outcomes daily with patient, family, and health team members.

Maintains safe practice by demonstrating unit specific clinical competency and demonstrating

compliance with all regulatory standards

Demonstrates safe practice in the administration of medications.

Ensures safe and effective handoff communications and documentation of such between care

givers.

Demonstrates sensitivity to pain management by alleviating and reducing pain to level

acceptable to patient.

The Advanced Critical Care Provider would be expected to complete the following under

physician direction and within the credentialing standards of the Hospital:

Assess patients for changes in condition and institutes appropriate interventions, including

initiation of necessary emergency care.

Procedures performed but not limited to: Placement of Chest Tubes, arterial lines and

catheters, central lines, venous catheters, pulmonary artery catheters, temporary

pacemakers, airway management, bronchoscopy, and defibrillation.

Participates and assists with the pre-, peri-, and postoperative management (which may

consist of: cardiac transplant, VAD, MCS, and ECMO patient in the and CVICUs) or other

areas in relation to critical care specialty.

Rotates through Neuro and Medical ICUs as well.

Preoperative:

The APRN/PA will perform initial interview and assessment of patient.

Assist in preoperative evaluation and education.

The APRN/PA will complete all clinical documentation for surgical patients, to include

consult notes, discharge notes, daily progress notes, and preoperative notes.

Communicates/coordinates with Pre-Admission Testing area, surgical scheduling, and

nursing units.

Communicates any abnormal findings to Chief of Anesthesia, Attending Surgeons for

inpatients and outpatients.

Order necessary testing or treatment within scope of license and protocols.

Implement treatment plans.

Perform patient education.

Communicate with all members of surgery, to include anesthesia providers and referring

surgeons.

Postoperative Hospitalization and Discharge:

Assists with postoperative management of patients under the direction of the Attending

Surgeon, executes a wide range of management plans for patients.

Discusses the care of each patient with the Attending Surgeon, and other consult

service(s) involved with the care of the patient.

Attends daily rounds and rounds independently on surgical patients as needed.

Works collaboratively with Case Management and prepares patients for discharge,

including patient and family discharge teaching, continuing care, and rehabilitation

arrangement in conjunction with nursing, writing discharge prescriptions, and confirming

primary care or referring physician follow-up.

Arranges for admission to the floor (H&P, orders).

Identifies patients for patient optimization, empowering patients in healthcare and

improving healthcare delivery.

Assist in compliance of Immediate Post-op notes prior to patient discharge from the PACU

department

Educational and Community Outreach

Assists with educational and community outreach as deemed necessary.

Coordinates, organizes, and participates independently in service line specific education

for patients

Schedule Details:

Ability to provide in-hospital coverage on a rotating shift basis.
