LPN/Licensed Practical Nurse Care Coordinator - 3 x 12's, 8am - 8pm 1,500 sign-on-bonus

Responsible for answering incoming telephone calls from patients/caregivers/medical professionals in a Call Center capacity.
Specialized work in receiving, screening and routing information or messages from the general public/facilities to necessary service areas.
Ability to react quickly and calmly, exercise good judgement and make sound decisions during urgent and routine communications.
Proficient in receiving, obtaining and relaying accurate information to be disseminated.
Provides skilled supportive patient and family care over the telephone in accordance to certification restrictions.
May be required to visit patients when needed.

Qualifications

• Current Florida LPN license required.
  
• Minimum of three years of nursing experience in a home health/hospice setting within the past five years is required.
  
• Knowledge of hospice, Medicare, Medicaid and private insurance benefits necessary
• Must be able to handle sensitive issues appropriately using sound judgment, maintain professional boundaries, and maintain confidentiality.
  
• Must be self-directed and able to work independently or as a team.
  
• Basic computer skills, use of common office software and EMR software experience.
  
• Excellent people and relationship-building skills.
  Excellent customer service skills.
  
• Maintain effective work relationships with co-workers, the public and administration.

• Responsible for upholding Treasure Coast Hospice/Model Standards for patient visits and compliant documentation.
  
• Understanding of and ability to execute routine Hospice Nursing:
  crisis intervention, communication skills, teaching skills, thanatology skills and needs.
  
• Under the supervision of the RN Case Manager, when needed, provide skilled professional nursing care to the patient and family unit incorporating physical, psychosocial, spiritual, cultural, and biological components.
  
• Utilize the nursing process, demonstrate accurate data collection, and assist in the planning, implementation and delivery of clinical care services.
  
• Implement the nursing plan of care in accordance to the physician orders and under the RN Case Manager.
  
• Provide appropriate pain and symptom management, including the patient's response to treatment and medications.
  
• Document patient care which reflects nursing intervention, patient's response to care and progress towards goals as set in the Plan of Care.
  
• Maintain clerical type records via the computer or handwritten data.
  
• Must demonstrate ability to provide an effective, supportive, sensitive approach in response to needs of patients, families and staff.
  Must be polite and courteous at all times.
  
• Provide program development strategies for Treasure Coast Hospice, in an organized, routine capacity that will enable the cohesion of team developments.
  Work collaboratively with other departments, demonstrate and encourage leadership for a comprehensive approach to program development.
