Description

New Horizons is the largest mental health and addiction recovery provider on the Treasure Coast (and beyond), serving 15,000 children and adults annually through inpatient crisis services, 24-hour help line and mobile response team, and outpatient programs conveniently located across Indian River, Martin, Okeechobee and St. Lucie counties. In addition, New Horizons assists 22,000 students in area schools, and we work closely with the courts, law enforcement, jails, and hospitals to help improve the health of individuals and the quality of life in our community.

New Horizons is a nonprofit agency seeking a caring and professional Dual Certified Psychiatric Detox/Medical Advanced Practice Registered Nurse (PMHNP/FNP) to fill a Full-time, Part-time, or PRN role.

As a Psychiatric and Medical APRN, you will provide services under the supervision of the designated medical director. The APRN shall provide the initial history and physical evaluations, on-going assessments, ordering and evaluating necessary lab tests, prescribing medications, alteration of medication, managing detoxification, providing appropriate medication management services for any patient requiring psychiatric intervention, provides appropriate medication management services for any patient requiring psychiatric intervention, establishing outreach/intervention assistance, and institute emergency certificates when appropriate. The ARNP will assess and monitor the client's physical condition and response to medications throughout the psychiatric stabilization and detoxification phases. The APRN is going to be seeing patients in the adults unit (SRT/ASU), Detox unit, and the Child and Adolescent unit.

The Advanced Practice Registered Nurse (APRN) follows the written established protocols of NHTC. Other regulatory agencies also have input into the scope of APRN practice and those policies and procedures shall be followed in respect to the APRN's assigned duties.

Detox/Medical Responsibilities include, but are not limited to:

  • Conduct physical exams, review medical histories, and identify medical issues for detox stays
  • Evaluates and provides medical management for detox/mental health clients
  • Ensure clients receive the appropriate level of medical detox care
  • Order diagnostic tests as needed
  • Provides case supervision on an on-going basis and medical follow-up as defined in established detox/medical protocols
  • Documents medication orders according to policies and procedures
  • Prescribes and adjusts medications following standards of care
  • Refer clients to hospitals or specialists when necessary
  • Monitor withdrawal symptoms and manage health needs
  • Review lab results and follow up on abnormal values
  • Participate in treatment planning and identify care needs during detox and post-discharge
  • Complete paperwork, maintain health records, and provide health education to clients and staff
  • Attends all physician staff meetings and rounds as requested
  • Maintains an active license meeting all licensure/educational requirements as defined by their Board of Licensure
Psychiatric/Medical Responsibilities include, but are not limited to:

  • Evaluates and provides diagnosis for mental health clients.
  • Formulates initial evaluation which includes:
  • Chief complaint
  • Psychosocial component
  • Psychiatric history
  • Mental Status exam
  • Diagnosis per DSM IV including all 5 Axis
  • Formulation of treatment plans which includes appropriate treatment modalities medication, lab work and referral for other medical orders are needed.
  • Provides case supervision on an on-going basis to provide medical follow up as defined in established psychiatric protocol.
  • Discharges patient with appropriate medication management and other appropriate aftercare services.
  • Follows the physician requested clinical duties as assigned.
  • Demonstrates the ability to order the right psychiatric medication in accordance with the patient's past psychiatric history and current behavior and mental status.
  • Documents the ordering of medications on the physician order form.
  • Utilizes medications from the Formulary when possible.
  • Monitors patient's progress and documents responses and reactions to the prescribed medication management program.
  • Assists in the documentation of all medication side effects or variances with the appropriate departments and committees.
  • Maintains current and up-to-date knowledge about all psychotropic and other psychiatric medications intended to assist the patient to a higher level of functioning.
  • Provides emergency psychiatric care as required.
  • Consults with psychiatrist as appropriate and receive clinical supervision whenever required by licensing board in the state of Florida.
  • Demonstrates an awareness of drug seeking behavior and prescribes medication only when clinically appropriate to treat an identified mental illness as listed in the Diagnostic and Statistical Manual of Mental Disorders-Fifth Edition.
  • Collaborate with counselors, social workers, psychologists, or other treatment team members to coordinate patient care
  • Conducts self-responsibly and demonstrates a high degree of dependability and autonomy as a member of the medical staff.
  • Attend all physician staff meetings and rounds as requested.
  • Works independently with a high degree of productivity.
  • Interacts and maintains effective and positive working relationships with clients, co-workers, and other mental and medical professionals.
  • Maintains an active license meeting all licensure/educational requirements as defined by their Board of Licensure.
  • Attend clinical supervision on an on-going basis with the Medical Director/ designee.
  • Provides appropriate clinical coverage for patients in his/her absence.
  • Performs other duties as requested.
  • Miscellaneous duties are performed according to instruction.
Physical Requirements
  1. Must be able to apply Therapeutic Effective Aggression Management (CPI/CARE) Techniques without limitations.
  2. Sedentary-Sedentary work involves sitting most of the time. Occasional walking or standing. Remaining in a seated position for an extended period.
  3. Job Function requires extended Standing or walking-Standing is to remain on one's feet in an upright position at various times to assist clients or staff.
  4. Lifting or carrying objects weighing 20-50 pounds
  5. Ability to lift or move objects weighing 51 pounds to 100 pounds with the use of appropriate devices
  6. The work requires visual acuity such as preparing and analyzing data and figures; utilizing computer systems; extensive reading.
