JOB SUMMARY
The Exercise Physiologist will conduct Coaching Sessions, Exercise Prescriptions and Health/Fitness Assessments for CHP members and patients. This position designs and monitors exercise programs for members and patients with various health conditions including cardiovascular disease, osteoporosis, diabetes, and other chronic conditions. The Exercise Physiologist collaborates with mid-level providers, nurses, physical therapists, and other professionals to ensure proper strengthening and conditioning of patients after they leave care.
Job Specific:

Conduct Coaching sessions, Exercise Prescriptions and Health/Fitness assessments for members. (EF)
Coordinate Medical Fitness programming for Physical Therapy and Cardiac Rehab patients after discharge, as well as Bariatric patients and other medical populations. (EF)
Coordinate physician and mid-level referrals for entry into our programs and services. (EF)
Instruct both individual and groups in exercise programming. (EF)
Teach health and wellness classes as needed. (EF)
Plan and implement member incentive programs in coordination with the Fitness Manager. (EF)
Offer personal training services for appropriate members as schedule and workflow allow. (EF)
Collaborate with nurses, physical therapists, and assistants regarding transition of patients. (EF).
Ensure that the fitness floor and all exercise areas are safe and free of hazards. (EF)
Conduct retention activities as directed and document these activities as required. (EF)
Assist with cleaning and maintaining equipment as requested.

Minimum Requirements
EDUCATIONAL REQUIREMENTS
Required: Bachelor's degree in Exercise Science or related field from an accredited university.
Preferred: Master's degree in same.
EXPERIENCE
Required: Minimum of one semester internship program in adult fitness and wellness environment.
Preferred: 1-2 years' experience working in an adult fitness, medical fitness, or rehab facility.
Special Skills and Abilities:
Required:
Excellent communication skills. Ability to teach and coach adults of various ages and backgrounds. Ability to relate to people from various socioeconomic backgrounds and cultures. Ability to demonstrate a wide variety of exercises. Detail oriented and organized and able to work with little supervision.
LICENSURE / CERTIFICATION REQUIREMENTS
Required: AHA HeartSaver First Aid/CPR/AED (Adult and Infant) upon hire OR within 30 days of hire. Water Safety training within 6 months of hire. NCCA accredited fitness certification upon hire OR within 6 months of hire.
Preferred: ACSM Exercise Specialist Certification
