Pride Health is hiring a Social Worker, Emergency Room to support our client's facility in Newark, DE. This is a Permanent, Full-Time (Possibility of extension or conversion) opportunity and a great way to start working with a top-tier healthcare organization!

Job Summary:

We are seeking an experienced Social Worker to join our Emergency Room Unit for a Permanent, Full-Time assignment. This position plays a critical role in providing care coordination and psycho-social support to patients in a fast-paced, high-acuity emergency care setting.

Key Responsibilities:

Work within a Care Management Triad Team model that includes RN Case Managers and Utilization Management staff.
Address psycho-social barriers to care and support discharge planning for emergency room patients.
Provide emotional support and crisis intervention for patients and families in the ER.
Collaborate with multidisciplinary teams to manage patient care, including assisting with the coordination of transfers and admissions.
Ensure a smooth discharge process, providing resources and referrals to community services as needed.

Qualifications:

Master's Degree in Social Work (MSW) or Master of Social Services (MSS) from an accredited program.
LCSW or LMSW certification required prior to start date.
Minimum 2 years of acute care experience (emergency room experience preferred but not required).
