Elwyn is seeking a dedicated and experienced

to join our growing ACT team in

Georgetown, DE .
As the Assertive Community Treatment (ACT) Program Nurse, you are a member of a multidisciplinary team, utilizing your nursing skill and experience to assess, plan, develop, coordinate, and provide treatment, rehabilitation, and support services to ACT clients under the clinical supervision of the team leader and the prescriber.

8 am - 4 pm, Monday - Friday
Responsibilities:
Clinical Care:
Provide person-centered, strength-based, trauma-informed care.
Implement the Prism Model for effective treatment of serious and persistent mental illness.
Conduct comprehensive assessments, including psychiatric and medical evaluations.
Administer psychotropic medications and monitor for effectiveness and side effects.
Develop, implement, and evaluate individualized treatment plans.
Provide evidence-based therapies (CBT-P, CBT, DBT, CAMS, MI).
Offer crisis intervention and de-escalation techniques.
Document client progress and activities in the EHR.
Provide crisis intervention and support during off-hours.
Case Management:

Coordinate and monitor client care, including medication management, medical appointments, and community services.
Advocate for client rights and preferences.
Assist with housing, benefits, and other practical needs.
Facilitate communication between clients, families, and other providers.
Team Collaboration:

Participate in team meetings and contribute to decision-making.
Collaborate with other team members to provide seamless care.
Provide supervision and training to team members as needed.
Administrative Duties:

Maintain accurate and timely documentation.
Manage medical supplies and medication inventory.
Adhere to regulatory standards and agency policies.
Participate in quality improvement initiatives.
Community Engagement:

Develop and maintain relationships with community providers.
Connect clients to appropriate community resources.
Promote recovery and wellness through community engagement activities.
Requirements:

Associate's degree in nursing, bachelor's degree, preferred.
3+ years in leadership, management role, experience in behavioral healthcare, and working with individuals with mental health conditions preferred.
Licensing in the State of Delaware as a Registered Nurse.
Preference will be given to candidates that meet the cultural and linguistic needs of the geographic area.
Current, valid driver's license.
