LOCATION: Sussex County Delaware (Seaford)

BROAD FUNCTION:

The Case Manager has the experience and a good command of the support and rehabilitation needs of adults with mental health, substance abuse, cognitive disabilities, and/or debilitating medical conditions. The Case Manager is responsible for overseeing the coordination of residential, psychosocial rehab, and personal care services for the residents.

The program aims to get individuals linked to housing, primary and behavioral healthcare, employment, and other community resources to improve the quality of their lives and ensure that positive outcomes are met. S/he is responsible for helping clients transition to stable housing and receive appropriate support to maintain housing and increase income.

Maintains an alliance with landlords, service recipients, housing, and treatment personnel, ensuring all participants are linked to all needed services and following up to ensure that services are accessed; coordinating the provision of medical services and behavioral health; coordinating overall independent living assistance services; working with community agencies to develop needed resources including housing, employment options, and income assistance; providing support and consultation with families; and providing supportive counseling.

PRINCIPAL DUTIES AND RESPONSIBILITIES:

Ensures that each care plan is resident-driven and identifies the needs, goals, and services that are being requested. Plans must include money management and coordination with other treatment providers and/or natural supports as needed.

Ensures crisis interventions are in place and communicated to all residents who may be experiencing a crisis.

Ensures that persons waiting for services at a higher level receive interim care that addresses their most acute concerns.

Works in conjunction with internal and external staff to ensure that all persons served to receive the services they need and achieve a seamless transition from one service level to the next in accordance with their needs.

Assists with the benefit application process and coordinating with appropriate State Service Centers, Social Security, ASSIST, and the Division of Disabilities Determination.

Ensures that data is collected for CMIS for each person served.

Ensures that the environment is safe, clean and free from hazardous conditions that could adversely affect the quality of life for the residents. This includes a monthly visit and inspection of all residences.

Ensures that the program follows Drug-Free Workplace requirements and other internal and external policies governing staff management.

Work with community and professional groups to ensure a mutually acceptable and amicable living environment.

Establishes and maintains relationships with reputable landlords.

Ensures that all contacts with residents and contact with residents' treatment providers and/or natural supports are notated.

Performs other duties as requested or assigned, verbally or in writing.

Daily contact with consumers and employees.

Frequent contact with treatment providers, family members, and other social supports of residents.

EDUCATION AND EXPERIENCE REQUIREMENTS:

A bachelor's degree in social work, sociology, or a similar degree is preferred. However, willing to train individuals working towards a bachelor's degree.

KNOWLEDGE, SKILL AND ABILITY REQUIREMENTS:

Knowledge of agency policies, procedures and operations.

Knowledge of Delaware landlord tenant code.

Knowledge of federal. State and local regulations including public health regulations and fire codes.

Skill in verbal and written communication.

Ability to establish and maintain outside agency and interdepartmental working relationships.

Ability to monitor for and relate deficiencies in public health and fire safety codes and provide and maintain clean, safe environment.

Recovery-orientation and ability to help residents maximize independence and self-sufficiency.

Ability to maintain confidentiality and exercise judgment tact, and diplomacy.

Valid Driver's License

ONGOING TRAINING REQUIREMENTS:

Certification in First Aid and CPR.

MANDT certification

Training in Defensive Driving

Training in Infection Control

Training in Suicide Risk Management

Training in Age and Cultural Competencies

Training about Agency policies and procedures on both a daily and emergency basis

Training in Fire Safety and Evacuation

Training in Safe Food Handling

Training about local resources in the community

Training in leisure activities

PHYSICAL DEMANDS:

Sitting, standing, walking, driving, and lifting.
