Per Diem Home Health Physical Therapist
We are seeking a Per Diem Home Health Physical Therapist to join our team at Constellation Health Services.
As a physical therapist with us, you will have the opportunity to make a meaningful difference in the lives of patients and their families.
The ideal candidate will be licensed to practice as a physical therapist in the state of Connecticut with at least one year of clinical experience, preferably in home care.
Oasis experience is strongly desired, but not required.
You will also be responsible for maintaining accurate and up-to-date patient records using our electronic medical record system.
In this role, you will:
• Develop and implement individualized treatment plans for patients based on their unique needs and goals.
• Educate patients and their caregivers on disease management, medications, and treatment plans.
• Conduct regular assessments and progress reports to ensure the effectiveness of treatment plans.
The successful candidate will possess excellent communication and interpersonal skills, with the ability to work effectively in a fast-paced environment.

We offer a competitive compensation package, including a salary range of $80,000 - $100,000 per year, depending on experience.
