GENERAL SUMMARY

Provides administrative support to program case management.

ESSENTIAL JOB FUNCTIONS*

Provides home visit relief to Nurses and Case Managers when needed.

Holds a small case load, and performances Case Manager role for that case load.

Audits charts for completion.

Completes and updates identification sheets and photos.

Follows up via telephone to request missing caregiver logs/financial transaction records (FTR) and other essential case file information.

Tracks six month summaries and physician's orders.

Tracks fire drills, home inspections, furnace inspections and safety plans.

Tracks caregiver trainings.

Files paperwork.

Tracks Individual Service Plans (ISP) and participant census via caregiver logs.

Tracks selected individuals' financial information (RFMS).

Serves as key contact for Adult Foster Care determination information.

Completes paperwork for new program participants/caregivers.

Ensures adequate supply of commonly used forms.

Manages incoming and outgoing mail.

Maintains daily staff schedule. Updates the on-call list.

Orders supplies.

Maintains updated relief list.

Calculates caregiver respite balances and submits check requests to management for approval.

Provides information and referrals.

Serves as mandated reporter for suspected abuse and/or neglect of individuals to the Disabled Persons Protection Commission (DPPC).

OTHER DUTIES AND RESPONSIBILITIES

Performs additional duties, as directed.

PREPARATION, KNOWLEDGE, SKILLS & ABILITIES

High school diploma.

Valid driver's license in the state of Massachusetts.

PC proficient.

Ability to work under pressure.

Ability to multi-task.

SUPERVISORY RESPONSIBILITY

No supervisory responsibility.

WORKING CONDITIONS/PHYSICAL DEMANDS

Ability to lift a minimum of 20 pounds in office supplies.
