JOB SUMMARY:
The Collaborative Care Provider (CCP) is responsible for coordinating and supporting the behavioral healthcare of patients with a specific role to provide collaborative care for common, less complex behavioral health conditions, such as anxiety or depression. The CCP maintains and tracks an active registry of patients, uses measurement-based care strategies to determine when treatment adjustments are needed, refer patients needing higher level behavioral health interventions if treatment not effective, engages in case management tasks, meets weekly with the psychiatric consultant and facilitates referrals to clinic sanctioned services within the community. The CCP is responsible for providing supportive patient interventions as defined in treatment team, patient safety plans, discharge planning, and other programmatic needs that support the success of the patient. The CCP provides documentation of patient services in the Electronic Medical Record and participates in the ongoing weekly psychiatric consultation of patient caseload.
SPECIFIC RESPONSIBILITIES:
Support and closely coordinate behavioral health care with the patient's primary care provider and, when appropriate, other mental health providers.
Outreach and schedule patients referred to collaborative care for both virtual, phone, and in-person sessions.
Participate in the behavioral treatment plan development of patient goals and ongoing reviews as indicated by patient, consulting psychiatrist, and primary care provider.
Screen patients for information regarding common mental health and substance use disorders.
Provide the patient with information and common mental health and substance use disorders along with available treatment options.
Implement brief interventions with patients that include goal setting, and for example behavioral activation, problem-solving, and motivational interviewing.
Participate in weekly caseload consultation with the psychiatric consultant to coordinate care and with medical providers and mental health providers as needed. Weekly one-hour consultation will include communication regarding treatment recommendations to the patient's PCP.
Support psychotropic medication management prescribed by PCPs by asking questions about treatment adherence, side effects, other complications, and effectiveness of treatment. Inform other members of the team if interventions are needed.
Administer, score, and document screening instruments for common mental health disorders. Utilize these measurement-based scores to track patients for changes in symptoms and gather information about treatment side effects or complications.
Utilize a registry to track patient progress in treatment. Registry documentation will include in-person and telephone/virtual encounters and will be utilized to identify and re-engage patients in Collaborative Care.
Provide ongoing supportive counseling and/or other recovery-oriented services as needed throughout the course of patient treatment.
Recognize professional limitations and scope of practice for role, referring patients to appropriate in-clinic or outside resources and high-intensity interventions as warranted. Follow-up with that patient to ensure they have connected and attended.
Ensure patient's aftercare needs are met by development and coordination of discharge planning.
Participate in ongoing utilization review tasks as needed in authorization of services.
Documentation of services in patient Electronic Medical Record.
Tracking time spent in collaborative care tasks over the calendar month and prepare record for billing and sign off by PCP.
Other program-oriented tasks will be required as needed such as ordering supplies, completing daily attendance sheets, etc.

EDUCATION AND EXPERIENCE
MINIMUM QUALIFICATIONS:
2 year degree in related field, plus experience with the population served, or experience and certifications related to substance use professional s, or Qualified Professional (QP) certification.
PREFERRED QUALFIFICATIONS:
4 years degree in social work or a related field plus experience working with similar populations.

00 pm (or flexed to best meet the needs of the clients and/or the Division); 40 hours per workweek; weekend, holiday, or evening coverage is occasionally required. Work hours will need to be flexible in order to respond to special work assignments, or evening activities, as requested by the team leader.
