Summary: Join our dedicated team as a Nurse Navigator specializing in Rheumatology, where you'll play a vital role in supporting patients as they navigate their healthcare journey. As a patient advocate and educator, you will help enhance their understanding of care options and improve their overall experience.
Responsibilities:
Regularly assess and review the progress of assigned patients, communicating the plan of care clearly to both patients and their families.
Provide emotional support, counseling, and clinical education to empower patients and families in their healthcare decisions.
Act as a key link between patients and clinical staff, administration, and external resources, ensuring smooth coordination of care and addressing any barriers encountered.
Assist patients with financial, psycho-social, functional, and administrative challenges by offering guidance and connecting them with appropriate resources.
Utilize critical thinking and decisive judgment skills to work effectively within a fast-paced environment, acting independently when necessary.
Requirements:
Minimum Required Education: Graduate of an accredited or state board-approved Registered Nursing program; a Bachelor's degree in Nursing is preferred.
Required Licensure/Certification: State licensure as a Registered Nurse (RN) and relevant life support certification.
Minimum Experience: 2 years of nursing experience.
BLS certification from the American Heart Association or the American Red Cross is required before the start date.
