Job Summary
The Case Manager I supports the clinical team by providing care planning, supporting service and community referrals. The Case Manager I supports Aftercare Services by helping to connect youth and family with community resources after discharge from residential care in order to ensure that educational, mental health, physical and psychiatric needs are met. Some Case Manager positions require meeting with and monitoring youth's successful engagement with their family and community for up to 6 months.
Essential Job functions
Deliver support and case management services to referred youth and families, including identification of available community-based programs and services.
Develop strong relationships and consistent communication with youth and families to enable ongoing connections with continuing care services.
Advocate for youth and families by liaising between the program and other internal and external resources.
Support phone and walk-in assessments as needed.
Triage support for assigned youth and families when required.
Develop and sustain productive working relationships with community service providers to promote a successful referral and service connection process for youth and families.
Perform consistent family/foster family meetings to ensure ongoing needs are being met and that any issues are addressed efficiently and effectively.
Maintain all necessary records, forms, reports, and summaries in client files according to agency/funder standards.
Transport youth to and from services and other community services to facilitate their ability to access services, when appropriate.
Support Aftercare Services by helping to connect youth and family with community resources after discharge from residential care in order to ensure that educational, mental health, physical and psychiatric needs are met.

Education & Experience
High School Diploma or GED required
Raise the Age Program: Bachelor's degree in Social Work or related field required
Minimum 2 years' experience in Human Services specializing in youth and family services required
Special Requirements
CPR certification required or obtained within 60 days of hire.
Unrestricted, valid NYS driver's license for minimum of 1 year with a clean driving record and minimum insurance coverage that meets agency standards.

Knowledge of community resources to support needs of youth and family.
Ability to complete all required documentation in timely fashion according to established policies and regulations. This is an essential function in order to facilitate the efficient and effective flow of information necessary for the program to be effectively managed and outcomes optimized.
Demonstrate problem-solving and conflict resolution skills.
Able to work independently and as part of an interdisciplinary team.
Meet the highest standard for ethical and professional conduct towards all.

The following physical demands are required:

Occasional balancing, bending, stooping, climbing, kneeling, pushing, pulling, reaching forward or down, reaching overhead, running, and twisting (up to 2. 5 hours)
Weekly lifting up to 50 lbs. (community-based staff may be required to lift an empty wheelchair weighing up to 50 lbs. ), 10-15 lbs. on a daily basis
Weekly carrying up to 55 lbs. approximately 10-100 ft. , 10-15 lbs. on a daily basis
Manual dexterity is required, including the frequent ability to grasp in both hands and continuous use of fine manipulation skills in both hands (approximately 1-2. 5 hrs. )
Occasional exposure to changes in temperature, dust, fumes, gases or chemicals is apparent
Protective clothing or equipment as required: Close toed shoes; Protective Shields, Universal Precaution PPE, Gloves for toileting and food preparation
Ability to change positions as needed
SPECIAL CONSIDERATIONS
May be required to physically restrain clients weighing between 50 and 300 lbs. and guide them safely to the floor. In some circumstances, restraints can last up to 15 minutes and may require repetition as necessary.
$18. 01 Minimum pay rate, $25. 75 Maximum pay rate, based on experience.
