• Addresses age-specific needs of adults and older adults.
• Provides quality care for cardiac rehab patients.
• Reviews and collects pertinent medical data on patients in Cardiac Rehabilitation Program.
• Assesses individual needs, and devises a pan of care to meet physiological, psychosocial and educational needs.
• Documents care provided.
• Documents observed responses to exercise program.
• Maintains communication with Primary Care Physician.
• Evaluates patients' progress in exercise and educational phases of program.
• Is responsible for safety and well being of the patient in an exercise environment.
• Other duties may be assigned.

Consistently provides service excellence to all patients, family members, visitors, volunteers and co-workers in a manner that reflects Falmouth Hospital's commitment to CARES: compassion, accountability, respect, excellence and service.
• Current MA Nursing Licensure required.
• Three years experience in coronary critical care required.
• Maintains BLS and ACLS.
