The Behavioral Health Specialist (BHS) provides day to day consultation with Lakeland Care's Care Management (CM) Supervisors and Interdisciplinary Team (IDT) staff to ensure delivery of effective care management services for members within every target group needing additional safety/crisis planning, positive behavioral support planning, risk mitigation or restrictive measure oversight.
This BHS will have a focus on working with members with complex mental health concerns, in the criminal justice system under Chapter 980/971, and will serve as a point of contact for external parties such as local law enforcement, county APS, DOC/DOJ, and community-based workgroups to support positive working relationships.
The Behavioral Health Specialist collaborates with interdisciplinary staff and Lakeland Care (LCI) providers to develop and maintain member centered, least restrictive, most inclusive, cost-effective care plans.
Position assesses critical aspects of care management as it pertains to member risk and positive behavioral support planning, developing resources, outreach to community resources in relation to crisis planning, and provide training to IDT staff and LCI providers as necessary.
• The position will require some travel to support members and the ideal candidate may reside within Dane County.
• Responsibilities & Competencies:
Serve as a resource and collaborate with IDT staff, natural and formal supports, to maximize multi-disciplinary, person-centered approaches to assist LCI members with complex care planning needs related to mental and behavioral health, crisis prevention, and crisis support and interventions in achieving positive outcomes.
Gather and evaluate information that leads to solid and sound decisions.
Identify obstacles and address problems cooperatively as they occur.
Promote a strong commitment to cross departmental teamwork.
Develop, implement, and train others in positive behavioral support planning, safety and emergency planning, crisis prevention, mental health services, and risk mitigation.
Participate in ongoing education and training to maintain knowledge base of areas of expertise as well as compliance with federal and state regulations.
Assist with reviewing solutions for gaps and/or quality concerns within the Provider Network as it relates to positive behavioral supports and/or risk mitigation.
Assist in reviewing for the proper utilization of related services including state centers and mental health facilities.
Participate in Lakeland Care's Restrictive Measures Review Committee, and other committees as appropriate.
Maintain the confidentiality of client information and protected health information as required by State and Federal regulations, including the Health Information Portability and Accountability (HIPAA) Act of 1996.
Requirements
Bachelor's degree in a health or human services related field, or equivalent combination of education and experience (includes knowledge, skills, and abilities).
A minimum of three (3) year's professional experience working with individuals with behavioral health, mental health, and with the criminal justice systems.
Three (3) years of experience working with one or more of the target groups served by Family Care.
Knowledge of community based long-term care and service delivery for the elderly, and individuals with physical, intellectual and/or developmental disabilities; knowledge and high level of understanding of the diagnosis associated with these target populations served by Lakeland Care.
Strong evaluation, assessment, research, and interview skills.
Strong knowledge of development and implementation of positive behavioral support plans, safety plans, and crisis plans.
Exceptional oral and written communication skills.
Enhanced ability to work independently with minimal oversight.
Working knowledge of basic computer programs and typing skills.
Current driver's license, acceptable driving record, and proof of adequate insurance required.
