• Trauma Level: I, II
• EMR: Cerner & SaAnesthesia
• Other Info:
• Locums do not medically direct CRNAs.
• 1 provider is assigned to OB each day (this will not be a locum)

• Active AZ License/IMLC
• BC/BE
• ACLS, PALS
• COVID & Flu vaccination proofs/exemption
