This state is part of the PT Compact privilege states 1 Year of experience Outpatient rehab experience BLS Bachelor's Degree in Physical Therapy is required.
Master's or Doctorate Degree in Physical Therapy is preferred Previous work experience providing direct patient care in outpatient, inpatient or inpatient rehab unit preferred Prior use of electronic medical records (EMR), preferred Floating may be required (60) miles of the original assignment location Details:
COVID-19 Initial Shot(s) All workers, including remote, must be fully vaccinated.
Fully vaccinated is defined as 2 weeks after the completion date of a o 1 or 2 dose monovalent vaccine series, or o 1 dose bivalent vaccine Exemption for Medical or Religious are reviewed by the client and must be completed using the Centura COVID Exemption form located in Egnyte.
Pfizer, Moderna, J&J, AstraZeneca and all World Health Organization accepted vaccine manufacturers are acceptable.
