The role will include assessments, diagnostics, and implementing a treatment plan whereby patients' conditions and plan of care can be met within their own home or within community settings to prevent them from remaining within the acute settings when deemed appropriate.Use of a car for transportation is essential.
We are looking for a dynamic, compassionate, organised, and confident person who has experience in managing acutely unwell patients, as there will be situations when patients require acute assessment, care, and escalation.
You will need to demonstrate the necessary skills to deliver services to patients with a wide range of complex clinical needs, including frailty.This role requires an active UK NMC Registration.Based Location:
