As a Trust, we are proudly signed up to the Disability Confident and Armed Forces Covenant guaranteed interview schemes.Job OverviewThis is an exciting opportunity for an Advanced Clinical Practitioner to join and assist with the development of the Hospital at Home service, alongside virtual wards specialities within Older Persons Services.
The role will include assessments, diagnostics, and implementing a treatment plan whereby patients' conditions and plan of care can be met within their own home or within community settings to prevent them from remaining within the acute settings when deemed appropriate.Use of a car for transportation is essential.
We are looking for a dynamic, compassionate, organised, and confident person who has experience in managing acutely unwell patients, as there will be situations when patients require acute assessment, care, and escalation.
You will need to demonstrate the necessary skills to deliver services to patients with a wide range of complex clinical needs, including frailty.This role requires an active UK NMC Registration.Based Location:
Christchurch Day Hospital but covering the whole of BCP area travelling.Interview Date:

Recruiting Managers interviewing for this role will be happy to answer any specific questions that you have at interview.Detailed and Main ResponsibilitiesTo understand the role in more detail, please read the full job description and person specification documents which are attached to this advert.Person SpecificationKnowledgeDemonstrable theoretical and practical knowledge of OPS specialty common presentations.Conversant in older persons Advanced practice developments at nation/local levels.Legal/professional/ethical issues of advanced practice.Advanced clinical skills, such as assessing patient's physical status requiring precision, accuracy, and dexterity.Demonstrable ability to work in a busy environment and resilience when the service is under pressure.Ability to analyse and interpret complex information from patient assessment and interventions, such as specified OPS rays, ECGs, and freedom to act in managing patients' care.Ability to work unsupervised and on own initiative and as part of a multi-disciplinary team.ExperienceDemonstrable experience of working independently at a Band 7 Clinical role within relevant specialty.Experience of leading & developing people.Demonstrates theoretical and practical knowledge OPS common presentations.Experience of teaching, mentoring, and developing staff.Managing defined caseload of patients.Experience of working across organisational boundaries.Publication/Conference Experience.Team leader.QualificationsProfessional registration with relevant professional body.Recognised teaching or mentoring qualification.Masters level Advanced Clinical Practitioner/Equivalent Masters.Non-Medical Prescribing.Budget Management.Recognition as Advanced Practitioner by the National Centre for Advancing Practice.Personal AttributesExcellent interpersonal and communication skills, using a range of methods, e.g., verbal, written, record keeping, complex report writing.Able to deal with demands of rapidly developing and changing service, problem-solving and prioritising abilities.
