Job Summary: Deliver medical care to LTC residents, manage chronic co-morbidities, collaborate with interdisciplinary teams, and provide high-quality customer service. Key responsibilities include clinical care, quality improvement, education, and leadership.
Key Requirements:
• Completion of recognized Nurse Practitioner program
• Current extended class certification with College of Nurses of Ontario
• Experience with chronic disease management and geriatric care
• Current vulnerable sector police check
• COVID vaccination
