December, 2024Reporting to the Manager of Allied Health Services, the Nursing Supervisor, @Home Programs, is responsible for the case management and nursing care of clients transitioning out of the hospital as part of Carefor's @Home bundled care programs.
The Nursing Supervisor is responsible for managing the wrap-around care that is coordinated and delivered to clients admitted to the program, with support from members of the care team.
Responsibilities include conducting InterRAI-HC assessments, designing care plans, engaging with other members of the care team (e.g., social workers, PTs, OTs, PTA/OTA, nutritional specialists, and speech language pathologists), and delegating tasks to personal support workers assigned to deliver care.
The Nursing Supervisor is also responsible for providing clinical support, identifying and following up with PSW job performance issues, participating in client case conferences, providing ongoing training and assessments, and providing feedback on performance.As a critical resource to the @Home Program, the Nursing Supervisor will support the care team and clients in providing safe, client-centred care by modeling practice standards, defining care processes and procedures, and collaborating in the design of personal support workers' education and training.
