As a Dietician at Champlain Rehab Solutions, you will assess and provide consultation, and recommendations of dietetic interventions to adults and/or geriatric group of clients in their homes using evidence-based best practice.
You will ensure coordinated delivery of care through ongoing communication with the client care team regarding plans and schedules for interventions, problem solve in collaboration with the client and care team to create opportunities for more independence and promote CRS's core beliefs and values.

Key areas of accountability include:

Preparing client reports for health professionals to analyze for treatment.
Preparing plans and presentations for clients and their caregivers to provide thorough information on their diet as well as their goals.
Educating internal and external staff members about healthy eating and proper nutrition.
Supporting the following areas of intervention:
failure to thrive, dysphasia, feeding problems, enteral feeding, diabetes, cancer, GI disorders, weight management, and palliative care.
Helping patients coming to terms with their illness and advising about special and time-specific diets.
Supporting safe client care through the development of knowledge and skills in accordance with our principles of continuous quality improvement.

Required qualifications include:

Member in good standing with the College of Dieticians of Ontario.
Minimum 2 years clinical experience (optimal).
Valid Driver's License with access to a vehicle.
Compassionate and empathetic.
Self-driven with ability to work independently.
Skilled in caseload management.
Skilled at electronic documentation.
Able to provide virtual care.
