OVERVIEW
The Clinical Nurse Specialist - Kidney Care Transition and Education works collaboratively with the Kidney Care Team to bridge patient care between the Multi-Care Kidney Clinic (MCKC) and other available treatment modalities to optimize nephrology patient outcomes, enhance safety and patient experience, and improve overall accessibility to care, reflecting the vision, mission, and values of Michael Garron Hospital, Toronto East Health Network.
Reporting to the Manager, Renal Program, the Clinical Nurse Specialist (CNS) - Kidney Care Transition and Education plays a lead role in collaborating between the kidney clinics, physicians, program staff, and patients to ensure continuity of care for nephrology patients.
The CNS demonstrates in-depth knowledge in kidney disease progression and management, and in the transition process to home modalities, kidney transplantation, or in-centre hemodialysis.
The CNS addresses complexities of kidney care through collaborative team consultation and builds capacity within the program.
The CNS assesses, plans, educates, implements, and evaluates the needs and assists patients and their families in navigating the kidney care journey by providing solutions and resources.
Moreover, the CNS supports the implementation, growth, and development of key renal program areas.
EDUCATION
Completion of a Bachelor's degree in Nursing required and Master's degree completed or in progress.
Registration with the College of Nurses of Ontario.
Specialty certification in Nephrology required.
Member of professional organization(s) in nursing such as RNAO or other relevant organizations.
Current CPR or BLS certificate required.
QUALIFICATIONS
Minimum 5 years relevant clinical specialty-related experience (kidney care) in an acute care hospital setting.
Demonstrated in-depth knowledge of kidney disease progression and in end-stage kidney disease management, including sound knowledge of home modalities such as peritoneal dialysis and home hemodialysis, and in the kidney transplantation referral and listing process.
Knowledge of the Ontario Renal Network.
Experience working in a leadership role an asset.
Evidence of sound knowledge and skill with adult education theory and techniques.
Demonstrated ability to work in a self-directed manner using well-developed leadership, critical thinking, organizational, and problem-solving skills.
Demonstrated ability to monitor and report on quality metrics and indicators.
Demonstrated strong interpersonal, team development, and conflict management skills.
Knowledge and skill in regard to patient safety, change leadership, project management, and evaluation an asset.
Demonstrated knowledge in research methods, and experience originating and conducting research an asset.
Knowledge of models of care regarding interprofessional practice and education.
Demonstrated flexibility to work occasional shifts and weekends to support operational needs.
Excellent time management and attendance record.
Proficient with Microsoft Office and other data processing software.
