Key Responsibilities:
As a Clinical Pharmacist with Lawtons, you will be responsible for providing exceptional patient-centered care and exceeding patients' expectations.
You will be part of a dedicated pharmacy team that is committed to providing comprehensive clinical services to continuing care facilities.
Responsibilities:
• Demonstrate warmth, empathy, and compassion while achieving excellence in patient centered care, customer service, workflow efficiencies, corporate pharmacy programs, and contractual requirements of continuing care facilities.
• Provide clinical services focused on continuing care contractual obligations, including disease state management, medication reviews, and full scope of practice, with reporting and after-hours emergency service as needed.
• Participate in professional programs and collaborate with colleagues to provide excellence in health and wellness services to all patients.
• Initiate care plans, follow up, and complete proper documentation, including identifying actual and potential drug-related problems through medication reviews.
• Educate nursing staff and participate on facility committees, completing biannual medication reviews as part of the care team with nurses, physicians, and nurse practitioners.
• Ensure prescription safety, prioritizing technical functions to pharmacy registered technicians and assistants as required, and provide quarterly reports to facility administration, reviewing all medication errors and completing audits.
Requirements:
• Bachelor of Science in Pharmacy from a recognized institution.
• Must be a licensed pharmacist, in good standing, in the province of responsibility.
• Injection and first aid certification, with additional certifications such as Certified Geriatric Pharmacist or Advance Prescribing preferred.
• Kroll software experience is a plus.
